How do I obtain my medical charts from a sleazy weight loss clinic ?
I cannot locate the Physician listed on my prescriptions -AND- the weight loss clinic was most likley involved in some fraudulent stuff, and I was to be one of their %26quot;patients%26quot; thanks in advance for any suggestions you may have.

you have an absolute right to your medical records.
For your protection, there are laws now that they can only be released with a written signature, so you can either sign a form that they provide you, or write up your own request, sign and date it.
i would walk in to the office, and wait while they made the copies. They do get to keep your originals, cause they have their own laws to follow, but they HAVE to release a copy to you.
if they give you grief, a lawyer would write a letter for you without much of a charge. I would also report them to the BBB for refusal to give you your records. if there are doctor%26#039;s there, you can report those doctors to the AMA too

If the clinic is open, just go in and ask for copies of them. You will have to sign a form for them, and if they are insistant that they are only used for further medical care, give them another Doctors name, but insist that you want to hand carry them, even though you wont. If that doesnt work, seek legal help. Good Luck!

ask your new doctor to send a record request that you have to sign...they then have the obligation to forward them (unless of course they were lost in a fire or something---which may happen)
you could go by in person, but they may not give them to you...even though they are yours, they are the responsibility of the clinic that you went to
